Triage Process:
● Intake worker may screen over phone or if you walk-in, an intake worker will help if
available to assist with mental health or substance use concerns.
● Process may take several hours.
● A sliding scale fee is applied for services rendered (i.e., triage clinic and ongoing
services)
● Bring insurance if current
● Bring list of current medications.
● Bring proof of residency (ID, recent letter with an address)
● If employed, bring in a pay stub, bank statement or social security benefit statement.
● Assistance for treatment possible depending on need and financial status
